26 Tex. Admin. Code § 282.57 - What information must the provider agency include in the request for a waiver?
The provider agency must include the following information for each staff member for whom it requests a waiver:
(1) name;
(2) job title;
(3) specific education or experience that
qualifies the staff member for the waiver; and
(4) a statement that he or she is competent
in providing services.
