26 Tex. Admin. Code § 320.137 - Transfer of Primary Responsibility for Individual in Restraint or Seclusion
(a) At the time of
transfer of primary responsibility between staff members for the individual in
restraint or seclusion, including transfer of responsibility at the change of
shift, the staff member with primary responsibility must meet with the staff
member who will assume primary responsibility to review the individual's
status. A staff member shall monitor the individual during the transfer
process.
(b) The review shall be
documented and shall include:
(1) information
regarding the time a restraint or seclusion was initiated;
(2) the nature of the circumstances requiring
restraint or seclusion;
(3) the
current status of the individual's physical, emotional, and behavioral
condition;
(4) any medication
administered; and
(5) the type of
care needed.
