26 Tex. Admin. Code § 558.214 - Notification Procedures for a Change in Agency Contact Information and Operating Hours
(a) An agency must report to HHSC no later than seven days after a change in the agency's:
(1) telephone number;
(2) mailing address, if different than the physical location; or
(3) operating hours.
(b) An agency must report the changes described in subsection (a) of this section to HHSC in accordance with §558.208 of this division (relating to Reporting Changes in Application Information and Fees).
(c) If an agency reports the information after the timeframes required by this section, the agency must pay a late fee as described in §558.208 of this division.
