28 Tex. Admin. Code § 21.3105 - Provision of CPT Codes

Each issuer of a health benefit plan subject to Insurance Code Chapter 1352 and this subchapter must, on request from the department, submit to the department the list of CPT codes identified by the issuer under § 21.3104(b)(1) of this subchapter.
