28 Tex. Admin. Code § 251.203 - Filing of Instruments

Any reference in this chapter to a specific form to be used for giving notices, making reports, or otherwise transmitting information to the commission is meant to include the form or forms prescribed for that particular purpose by the commission's executive director, pursuant to the Texas Workers' Compensation Act, as of the time that the notice, report, or transmittal is required to be made. The following shall be filed with the State Office of Risk Management in Austin:

(1) Employer's First Report of Injury or Illness (Form TWCC-1S);

(2) any supplemental report of injury by the employing agency (Form TWCC-6);

(3) any special reports required by the director;

(4) any other forms prescribed or required from the employing agency by the commission will be submitted to the director who will be responsible for filing with the commission.
