28 Tex. Admin. Code § 34.519 - Installation Labels for Fixed Extinguisher Systems
(a) After an installation has been completed,
an installation label must be affixed to the control head or panel of the fixed
fire extinguisher system. The signature of the licensee on the label certifies
that the system has been installed according to law. Labels must be five inches
in height and four inches in width and must be of the gum label type. They must
not be red in color. Installation labels must contain only the following
information in the format of the label shown in subsection (b) of this section:
(1) the inscription "DO NOT REMOVE BY ORDER
OF THE STATE FIRE MARSHAL--SYSTEM INSTALLATION RECORD" (all in capital letters,
at least 10-point boldface type);
(2) the firm's name, address, and telephone
number;
(3) the firm's
certificate-of-registration number;
(4) the signature and license number of the
licensee authorized to certify a fixed fire extinguishing system (a stamped
signature is prohibited);
(5) the
date of installation; and
(6)
identification of the manufacturer's manual(s) used for installation.

(d)
After completion of the installation, modification, or addition of a fixed fire
extinguisher system, the licensee must complete an installation certificate in
the format provided by the state fire marshal (see Certificate of
Installation). When an installation certificate has been completed, legible
copies must be distributed as follows:
(1)
original to owner or posted on site at control head or panel;
(2) a copy to main authority having
jurisdiction, if required; and
(3)
a copy to certifying firm to retain in their office for access by
SFMO.
