34 Tex. Admin. Code § 13.6 - Minimum Requirements for a Claim
A claim under Property Code, Chapter 74, Subchapter F, must identify:
(1) the name of each
claimant;
(2) the mailing address
for each claimant;
(3) the social
security number or employer identification number of each claimant, or a
statement that a claimant does not have such a number; and
(4) each specific property in the possession
of the comptroller that is being claimed by reference to the unique property
identification number assigned to each claimed property by the
comptroller.
