40 Tex. Admin. Code § 700.892 - What documents must an adoptive parent submit to DFPS to establish initial eligibility for the HCB stipend?
(a) An adoptive
parent must submit:
(1) A final adoption
order;
(2) Proof of income, such as
a W-2 form, verification of earnings statement from an employer, or other
reliable source;
(3) Proof of
denial of medical assistance benefits under Chapter 32 of the Human Resources
Code; and
(4) Proof that the:
(A) Child in question currently has health
coverage benefits paid for by the adoptive parent, and the amount of the
monthly premium; or
(B) Adoptive
parent has obtained a quote for health coverage benefits that will be obtained
for the child in question following receipt of the stipend; and
(b) A parent who
provides a quote for health care coverage as provided under subsection
(a)(4)(B) must provide proof that the parent obtained health care coverage for
the child in question within 30 days of the date on which the first HCB stipend
payment is issued.
