(1) As required by
Section
R13-2-4, a
request is submitted through the Open Records Portal. No other form is
required.
(2) An appeal form may be
acquired from https://archives.utah.gov/rim/forms/forms-grama.html [File Link
Not Available]. An appellant is not required to use an appeal form so long as
information required under Subsection
63G-2-401(2)
is provided.
