Utah Admin. Code R156-1-308k - Relicensure Following Surrender of Licensure - Requirements
(1) An applicant who surrendered a license
that was active and in good standing when it was surrendered shall meet the
requirements for licensure in Sections
R156-1-308a through
R156-1-308l.
(2) An applicant who surrendered a license
while the license was active but not in good standing as evidenced by the
written agreement supporting the surrender of license shall:
(a) submit an application for licensure
complete with supporting documents as is required of an individual making an
initial application for license demonstrating the applicant meets the current
qualifications for licensure and compliance with requirements and conditions of
license reinstatement;
(b) pay the
license fee for a new applicant for licensure;
(c) provide information requested by the
Division and board to clearly demonstrate the applicant is currently competent
to be relicensed to engage in the profession; and
(d) pay any fines or citations owed to the
Division.
