Utah Admin. Code R156-17b-401 - Disciplinary Proceedings
(1) An individual
licensed as a pharmacy intern who is currently under disciplinary action and
qualifies for licensure as a pharmacist may be issued a pharmacist license
under the same restrictions as the pharmacy intern license.
(2) A pharmacist, pharmacy intern, pharmacy
technician, pharmacy technician trainee, or DMP whose license or registration
is suspended under Subsection
58-17b-701(6) may
petition the Division at any time to demonstrate the ability to resume
competent practice.
