Utah Admin. Code R156-37-302 - Qualifications for Licensure - Application Requirements
(1) An applicant for a controlled substance
license shall:
(a) submit an application in a
form prescribed by the Division;
(b) pay the fee established by the Division
under Section
63J-1-504; and
(c) be currently licensed in good standing by
the state in a classification in Section
R156-37-301.
(3) The Division and the reviewing board may
request from the applicant information that is reasonable and necessary to
permit an evaluation of:
(a) the applicant's
qualifications to engage in practice with controlled substances; and
(b) the public interest in the issuance of a
controlled substance license to the applicant.
(4) To determine if an applicant is qualified
for licensure, the Division may:
(a) assign
the application to a qualified and appropriate licensing board for review and
recommendation to the Division; and
(b) conduct site inspections, review research
protocol, conduct interviews with persons knowledgeable about the applicant,
and conduct any other investigation that is reasonable and necessary to
determine the applicant is qualified to receive a controlled substance
license.
