Utah Admin. Code R156-60a-303 - Renewal and Reinstatement of License
(1) Under Subsection
58-1-308(1), the
renewal date for the two-year renewal cycle for all licenses under Title 58,
Chapter 60, Part 2, Social Worker Licensing Act, is established in Subsection
R156-1-308a(1).
(2) Renewal and reinstatement
procedures shall be in accordance with Sections
R156-1-308a through
R156-1-308l, except as provided
in Subsection (3).
(3) Under
Subsection
58-1-308(5)(a)(ii)(B)
and Section R156-1-308g, an applicant for
reinstatement of licensure as an LCSW whose license has been expired over five
years, shall upon the Division' request:
(a)
meet with the Board to evaluate the applicant's ability to safely and
competently practice clinical social work; and
(b) complete one or more of the following:
(i) establish a plan of supervision under an
approved supervisor, which may include up to 3,000 hours of clinical social
work and mental health therapy training as a CSW before qualifying for
reinstatement of the LCSW license;
(ii) pass the ASWB Clinical Licensing Exam;
or
(iii) complete up to 40 hours of
continuing education in subjects determined by the Board.
