Utah Admin. Code R21-1-10 - Original Signature Required on Certain OSDC Documents
OSDC requires an original signature on:
(1) a victim settlement agreement;
(2) a payment agreement;
(3) a wage assignment;
(4) an authority for the automatic transfer
of funds (EFT); and
(5) an
authority for the automatic credit or debit card charge.
