Utah Admin. Code R21-4-4 - Notice to Debtors
As stated in Subsection 63A-3-508(4), OSDC shall make a reasonable effort to identify and send written notice to any debtor who may be eligible to receive credit.
(1) The notice does not guarantee a debtor
will receive credit.
(2) The notice
shall be sent by first-class mail to the debtor's last known address.
(a) Any notice returned to OSDC that has a
forwarding address shall be noted, and another notice shall be sent to the
forwarding address.
(b) Any notice
returned without a forwarding address shall be noted and no further notice
shall be sent.
(3) The
notice shall contain language that states:
(a)
that the debtor may be eligible for a credit toward amounts still owed
according to the eligibility requirements listed in Section R21-4-6;
(b) that receiving the notice does not
guarantee the debtor will receive credit; and
(c) that the debtor may choose to send a
request for review. The notice shall include the list of requirements for
sending a request for review as listed in Section R21-4-5.
