Utah Admin. Code R311-500-6 - Certification
(a) Initial certification for all certificate
holders shall be effective for a period of two years from the date of issuance,
unless revoked before the expiration date pursuant to
R311-500-9.
Certificates shall be subject to periodic renewal pursuant to
R311-500-7.
