Utah Admin. Code R315-310-6 - Contents of a Permit Application for a New or Expanding Landtreatment Disposal Facility
(1) Each
application for a landtreatment disposal facility permit shall contain the
information required in Section
R315-310-3.
(2) Each application for a permit shall also
contain:
(a) a geohydrological assessment of
the facility site that addresses all of the factors of Subsection
R315-310-4(2)(b);
(b) engineering report, plans,
specifications, and calculations that address:
(i) how the proposed facility will meet the
location standards pursuant to Section
R315-302-1;
(ii) how the proposed facility will meet the
standards of Rule R315-307;
(iii)
the basis for calculating the facility's life;
(iv) waste analyses and methods to
periodically sample and analyze solid waste;
(v) design of interim waste storage
facilities;
(vi) design of run-on
and run-off control systems;
(vii)
a contour map of the active area showing contours to the nearest
foot;
(viii) a ground water and
surface water monitoring program; and
(ix) access barriers such as fences, gate,
and warning signs.
(c) a
plan of operation that in addition to the requirements of Section
R315-302-2
addresses:
(i) operation and maintenance of
run-on and run-off control systems;
(ii) methods of taking ground water samples
and for maintaining ground water monitoring systems; and
(iii) methods of applying wastes to meet the
requirements of Section
R315-307-3.
(d) closure plan to address:
(i) closure schedule;
(ii) capacity of site in volume and tonnage;
and
(iii) final inspection by
regulatory agencies.
(e)
post-closure plan to address:
(i) estimated
time period for post-closure activities;
(ii) site monitoring of ground
water;
(iii) changes in record of
title;
(iv) maintenance activities
to maintain cover and run-off system;
(v) plans for food-chain crops, if any, being
grown on the active areas, after closure; and
(vi) identification of final closure costs
including cost calculations and the funding mechanism.
