Utah Admin. Code R317-12-5 - Revocation of Certification and Appeal
5.1 Revocation of prior certification shall
be made for any of the circumstances prescribed in Section
19-12-304.
5.2 An appeal of a denial of certification or
a revocation of certification by the Director may be contested by filing a
Request for Agency Action as provided in Rule R305-7.
