Utah Admin. Code R457-1-612 - Boxing. Identification - Photo Identification Cards
(1) Each contestant shall provide two pieces
of identification to the designated Commission member before participation in a
fight. One of the pieces of identification shall be a recent photo
identification card issued or accepted by the Commission when the contestant
receives the contestant's original license.
(2) The photo identification card shall
contain the following information:
(a) the
contestant's name and address;
(b)
the contestant's social security number;
(c) the personal identification number
assigned to the contestant by a boxing registry;
(d) a photograph of the contestant;
and
(e) the contestant's height and
weight.
(3) The
Commission shall honor similar photo identification cards from other
jurisdictions.
(4) Unless otherwise
approved by the Commission, a contestant may not compete if the contestant's
photo identification card is incomplete or if the contestant fails to present
the photo identification card to the designated Commission member before the
bout.
