Utah Admin. Code R501-13-2 - Record Keeping
(1)
(a) In addition to record keeping
requirements in Rule R501-1, the director shall maintain the following onsite
at all times:
(b) daily client
attendance records; and
(c) a
current health assessment signed by a physician for each client.
