Utah Admin. Code R512-308-12 - Appeals and Fair Hearings
(1) When a
decision is made to deny, reduce, or terminate a guardianship subsidy, Child
and Family Services shall send by certified mail a written Notice of Agency
Action.
(2) The notice shall
include information about how to request a fair hearing.
