Utah Admin. Code R590-287-7 - Patent History
The reported patent history shall include the following
information for a manufacturer's drug that lost patent exclusivity in the
United States during the three calendar years prior to the effective date of
the reported cost increase for the
(1) the name of the drug;
and
(2) the date when the patent
expired.
