Utah Admin. Code R590-70-9 - Acquisition of Control - Statement Filing (Form A)
(1) A person required to file a statement
under Section
31A-16-103
shall furnish the required information on Form A.
(2) The person in Subsection (1) required to
file a F o r m A shall also furnish the required information on Form E, as
described in Section
R590-70-12.
