Utah Admin. Code R623-8-12 - Documenting Problems
Any documentation required in this rule shall include any reporting problems or irregularities, and, if applicable shall include:
(1) details of any observed issues or
problems;
(2) the date and time of
when issues or problems occurred;
(3) any action taken in response to issues or
problems; and
(4) any resolution to
issues or problems.
