210. Request for
Agency Action (petition) will contain a title which will be substantially in

Docket and Cause Number. Upon the filing of a Request for Agency Action
(petition), the secretary of the Board will assign a docket and a cause number
to the matter. The secretary will enter the docket and cause numbers for the
matter, together with the date of filing, on a separate docket provided for
that purpose. Thereafter, all pleadings offered in the same proceeding will
bear the docket and cause numbers assigned and will be noted with the filing
date upon the docket page assigned.
230. Content and Size of Pleadings. Pleadings
should be double-spaced and typed on plain, white, 8-1/2" x 11" paper. They
must identify the proceeding by title and by docket and cause number, if known.
All pleadings will contain a clear and concise statement of the matter relied
upon as a basis for the pleading, together with an appropriate prayer for
relief when relief is sought.

Amendments to Pleadings. The Board may, upon motion of the responsible party
made at or before the hearing, allow any pleadings to be amended or corrected.
Defects which do not substantially prejudice any of the parties will be
disregarded.
250. Signing of
Pleadings. Pleadings will be signed by the party or the party's attorney and
will show the signer's address and telephone number. The signature will be
deemed to be a certification by the signer that he or she has read the pleading
and that, he or she has taken reasonable measures to assure its
truth.
