Utah Admin. Code R649-8-19 - UIC Form 2, Monthly Report of Enhanced Recovery Project
1. The operator shall submit this report
monthly to report the injection pressure, rate, and volume for each enhanced
recovery injection well or project.
2. The report is due within 30 days following
the end of the month of operations.
