Utah Admin. Code R649-8-20 - UIC Form 3, Monthly Injection Report
1. The operator shall submit this report
monthly to report the daily injection pressure, rate, and volume for each
disposal well and/or storage well.
2. The report is due within 30 days following
the end of the month of operations.
