Utah Admin. Code R649-8-21 - UIC Form 4, Annual Fluid Injection Report
1. The operator of disposal wells, storage
wells, or enhanced recovery projects shall file an annual report with the
division using this form.
2. The
report is due within 60 days following the end of the year.
