Utah Admin. Code R911-3-11 - Change of Owner
(1) A licensed
provider cannot transfer their license and the vehicle permits to another
party.
(2) A new owner shall submit
within ten calendar days before the acquisition of property, applications and
fees for a new license and vehicle permits.
