Utah Admin. Code R986-200-209 - Participation in Obtaining an Assessment
(1) Within 30 business days of the date the
application for financial assistance has been completed and approved, the
client will be assigned to an employment counselor and must complete an
assessment.
(2) The assessment
evaluates a client's needs and is used to develop an employment plan.
(3) Completion of the assessment
requires that the client provide information about:
(a) family circumstances including health,
needs of the children, support systems, and relationships;
(b) personal needs or potential barriers to
employment;
(c) education;
(d) work history;
(e) skills;
(f) financial resources and needs; and
(g) any other information relevant
to the client's ability to become self-sufficient.
(4) The client may be required to participate
in testing or completion of other assessment tools and may be referred to
another person within the Department, another agency, or to a company or
individual under contract with the Department to complete testing, assessment,
and evaluation.
