Utah Admin. Code R986-600-657 - Applying for Continued Eligibility Training Provider and Program Eligibility
(1) A training
provider must certify that all the information previously provided for each
program for which they are seeking continued eligibility is current and
correct, or update program information for each program, including:
(a) changes in the availability of the
programs being offered; and
(b)
changes in the cost of attendance, including changes to tuition, fees, or any
other costs imposed by the training provider.
(2) As part of continued eligibility a
provider must submit performance data by program according to the deadlines set
by the Department, including aggregate data for any student participating in or
attending ETPL-approved programs.
(3) The Department will also consider a
provider's past compliance with the training provider Terms and Conditions
Agreement when determining continued eligibility.
(4) A program that does not meet the minimum
standards or provide the required information by the renewal date will be
removed from the ETPL. If a provider cannot complete the renewal requirements,
an extension may be granted if the delay is due to exceptional circumstances or
circumstances that are beyond the provider's control. The request for an
extension must be submitted 30 days before the renewal deadline or as soon as
possible.
(5) The Department will
notify the training provider of the decision on continued eligibility. If an
application is denied, the notification will include information on the appeals
process as described in Section
R986-600-659.
