13-012 Code Vt. R. 13-140-012-X - DRUG FORMULARY
Section 1
A new rule entitled "Generic Formulary" is added to read:
§ FORMULARY
(a)
The Vermont Formulary Committee, hereinafter referred to as "committee", hereby
adopts the list of drugs certified by the federal Food and Drug Administration
as being safe, effective, and therapeutically equivalent; including all future
amendments.
This list of non-substitutable products will also be updated, based on material received from F.D.A. and the opinion of the Committee as provided by 18 VSA Chapter 89.
(b) This rule does not cover single-source
products, over-the-counter preparations, diagnostic agents, or nutritional
supplements.
§ CONTENT, STRUCTURE, AND FORMAT
(a) Drugs are listed along with their dosage
forms and strengths. They are listed alphabetically by generic and brand name.
Only approved manufacturers are listed for each drug.
(b) At the end of the formulary, an
alphabetical index has been provided which lists the page numbers on which any
specific brand name or generic name can be found.
(c) For antibiotic drugs which require
batch-by-batch certification, the strengths have been purposely
omitted.
(d) Manufacturers names
have been shortened, and abbreviations have been used for the various dosage
forms.
(e) This list will be
distributed to all registered pharmacies and prescribers in Vermont and to
various State agencies and departments. Future updates will be distributed to
the same individuals and organizations.
§ APPROVED DISTRIBUTORS: PROCEDURES
(a) Numerous approved distributors exist for
many products listed in the formulary. Provided their products are made by
approved manufacturers, they too are acceptable.
(b) When purchasing a drug product from a
distributor, make sure the distributor holds an F.D.A. approved distributor
supplemental application for that particular drug product and dosage
form.
