Wis. Admin. Code Medical Examining Board Med 22.03 - Applications and credentials
Every applicant for initial licensure as a perfusionist shall submit all of the following:
(1) A
completed application on a form provided by the board.
(2) The fee specified in s.
440.05,
Stats.
(3) Satisfactory evidence
that the applicant has successfully completed an educational program in
perfusion recognized by the board and accredited by the Accreditation Committee
for Perfusion Education of the Commission on Accreditation of Allied Health
Educational Programs.
(4) Evidence
the applicant has passed the examinations required under s.
Med 22.04.
