Wis. Admin. Code Department of Natural Resources NR 149.07 - Transfer of certification and registration
(1) Laboratory
certifications and registrations are not transferable to other entities unless
the department expressly approves the transfer.
(2) Laboratories shall notify the department
of any change of ownership as soon as practicable, but no later than 30 days
after the change has occurred. As part of the notification, the laboratory
shall provide the department the number of analytical staff working or expected
to be working at the facility 30 days before and after the ownership
change.
(3) The department shall
inform the laboratory within 30 days after the receipt of the notification or
the actual transfer of ownership, whichever happens later, whether the
laboratory is eligible for having existing certifications or registrations
transferred by application, or whether an initial application is required to be
submitted by the new laboratory owner.
(a)
The laboratory shall submit the type of application the department has
determined is appropriate within 30 days after the date of the determination
notification.
(b) All
certifications and registrations granted to the laboratory changing ownership
shall expire 30 days after the department notifies the laboratory of the type
of application required to be submitted.
