048-47 Wyo. Code R. §§ 47-10 - Individualized Plan of Care
(a) An
individualized plan of care (POC) shall be developed with the Family Care
Coordinator (FCC), family, youth, and family team (CFT) within thirty (30) days
of selection of an FCC.
(b) A
crisis plan shall be included as part of the POC to assist in stabilizing the
youth and family during a crisis.
(c) The POC shall be updated at least every
ninety (90) days. More frequent updates may be necessary depending on the needs
of the youth and his or her family.
(d) Each youth's PCP shall be documented in
the POC.
