065-4 Wyo. Code R. §§ 4-6 - Change of Name, Address or Telephone Number
(a) Licensees shall
register with the Board any change in their legal name, shall submit
documentation demonstrating the change of name, appropriate fee and shall
surrender the old wall certificate and a new wall certificate shall be issued
by the Board.
(b) Each applicant
and licensee shall file with the Board their current home and professional
mailing addresses and telephone numbers and shall report to the Board in
writing any change of addresses or telephone numbers, giving both old and new
addresses and telephone numbers.
