251-8 Wyo. Code R. §§ 8-5 - License by Endorsement
(a) Applicants who have for the last five (5)
years preceding the application been a practicing veterinarian licensed in a
state, territory, or district of the United States having license requirements
at the time of application in that jurisdiction that were substantially
equivalent to the requirements of the Act and these rules shall submit:
(i) A completed application for license on
forms provided by the Board accompanied by the required application
fee.
(ii) Verification of licensure
from all jurisdictions in which the applicant has held or currently holds a
license or permit.
(iii) An
affidavit that no complaint has been filed or is pending, no investigation is
pending and no disciplinary action has been taken or is pending against the
applicant in any jurisdiction.
