Ariz. Admin. Code § R21-6-405 - Home Study and Assessment
A.
The applicant and adult household members shall complete self-assessments,
using the forms approved by OLR, and share the results of the self-assessments
with the licensing agency. The licensing agency shall, in the home study,
summarize and consider information provided in the self-assessments.
B. All household members, including each
child household member, if appropriate to the child's age and developmental
level, shall participate in interviews conducted by the licensing agency, as
directed by R21-6-206.
C. The
applicant shall participate in and successfully complete pre-service training
as specified in R21-6-303.
D. The
applicant shall provide additional information as needed for the licensing
agency to evaluate the fitness of the applicant and to conduct the home
study.
