Ariz. Admin. Code § R3-2-1119 - Mutual Termination
The Department and the recipient of service may mutually agree to termination of the service, under the following terms:
A. Previously paid fees will not be returned
to the service recipient.
B.
Pending charges will be paid in full for completed work of the
Department.
C. A pending
application will be considered terminated, but a new application may be filed
at any time, without penalty.
D.
Termination shall not take effect until the end of a 30-days' notice period,
unless the parties agree otherwise.
E. The mutual decision to terminate and any
related agreements are documented in writing.
