Ariz. Admin. Code § R4-23-676 - Third-party Logistics Provider Permit
A. A person shall not provide logistics
services, as described under A.R.S. §
32-1941(A), until the Board issues a third-party logistics provider permit for the
facility.
B. A person that wants to
provide logistics services shall obtain a Board-issued third-party logistics
provider permit for each facility.
C. Application. To obtain a third-party
logistics provider permit for a facility, a person shall submit a completed
application, using a form available on the Board's website, and the fee
specified in
R4-23-205.
D. Change of ownership. A third-party
logistics provider permittee shall comply with R4-23-601(F).
E. A third-party logistics provider permittee
shall renew the permit as specified under R4-23-602(D).
F. The Board shall adhere to the time frames
specified under
R4-23-602(C)
when processing an initial or renewal application for a third-party logistics
provider permit.
