Ariz. Admin. Code § R6-13-104 - Applicant Responsibilities at Initial Application
A. A person shall apply for TC payments by
submitting an identifiable Department-approved application to an FAA office in
person, by mail, fax, or electronic transmittal.
B. An identifiable application means an
application that contains:
1. The legible
name and address of the applicant; and
2. The signature of the applicant, the
applicant's representative, or if the applicant is incompetent or
incapacitated, someone legally authorized to act on behalf of the
applicant.
C. The
application filing date is the date an FAA office receives an identifiable
application. If the applicant is eligible, the Department shall pay TC payments
calculated from this date.
