Ariz. Admin. Code § R6-13-105 - Department Responsibilities at Initial Application
A. Upon receipt of an identifiable
application, the Department shall:
1. Date
stamp the application with the application filing date, and
2. Schedule an initial eligibility interview
with the applicant at:
a. A location that
ensures a reasonable amount of privacy, or
b. A homebound applicant's residence,
or
3. Schedule a
telephone initial eligibility interview.
B. The Department shall assist the applicant
in completing the application if necessary. A completed application shall
contain:
1. The names of all persons living
in the applicant's dwelling and their relationship to the applicant,
2. A request to receive TC payments,
and
3. All financial and
nonfinancial eligibility information requested on the application
form.
