Ariz. Admin. Code § R6-6-105 - Consent for Release of Information
A. Consent for the release of personally
identifiable information shall be:
1.
Obtained from the client or responsible person in writing and dated;
2. Maintained in the main record.
B. Consents for release of
information obtained during intake shall expire within 90 days.
C. Subsequent consents shall be obtained as
needed and shall be valid for six months from the date of execution.
