(1) For licensure
by examination, the examination fee is $250.00. This fee is in addition to the
fee charged for the NHA and CORE examinations as stated in rule
64B10-11.002(1)(a), F.A.C.
(2) The
reexamination fee is $250.00.
(3)
The initial licensure fee, whether by examination or endorsement, is $500.00.
An applicant not eligible for licensure may receive a refund of the initial
licensure fee.
(4) The application
fee for a temporary license is $325.00
(5) The licensure fee for a temporary license
is $150.00.
(6) The application fee
for the 1,000 hour administrator-in-training program is $250.00.
(7) The application fee for the 2,000 hour
administrator-in-training program is $350.00.
(8) The fee for preceptor initial
certification is $100.00.
(9) The
fee for renewal of an active license is $325.00
(10) The application fee for inactive status
is $325.00.
(11) The fee for
renewal of an inactive license is $325.00.
(12) The fee for reactivation of an inactive
license is $325.00.
(13) The
retired status fee is $50.00.
(14)
The fee for reinstatement of a delinquent license is $325.00.
(15) The change of status fee is
$25.00.
(16) The application fee
for continuing education course providers is $100.00.
(17) The biennial renewal fee for continuing
education course providers is $50.00.
