Fla. Admin. Code Ann. R. 64B11-2.002 - Admission by Endorsement

An applicant for licensure by endoresment pursuant to Section 456.0145(2), F.S., shall submit his or her application DH-MQA 5101 (eff. 09/24), "Mobile Opportunity by Interstate Licensure Endorsement" (MOBILE) which is incorporated herein by reference and which may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-17205, the Board office, or at https://floridasoccupationaltherapy.gov/. Applicants for licensure shall meet and comply with all requirements in Section 456.0145(2), F.S.
