Fla. Admin. Code Ann. R. 64B12-12.008 - Reactivation of an Inactive License
An inactive status license may change to active status at anytime provided the licensee:
(1)
Submits a written request for reactivation;
(2) Demonstrates compliance with the
continuing education requirements of Rule
64B12-15.001, F.A.C., for the
biennium the license was in inactive status;
(3) Pays the reactivation fee set forth in
Rule 64B12-11.010, F.A.C.;
and,
(4) Pays the applicable
renewal fee:
(a) If the reactivation request
is received during the license renewal cycle, pays the active renewal fee set
forth in Rule 64B12-11.003, F.A.C.,
or
(b) If the reactivation request
is received at any other time than at the time of license renewal, pays the
difference between the inactive renewal fee and the active renewal fee and the
change of status fee set forth in Rule
64B12-11.0105,
