Fla. Admin. Code Ann. R. 64B16-26.1022 - Permit Fees
(1) The initial
permit fee for a pharmacy, as provided by Section
465.022(14)(a),
F.S., shall be $250.00.
(2) The
biennial permit renewal fee for a pharmacy, as provided by Section
465.022(14)(b),
F.S., shall be $250.00.
(3) The
change of location fee for a pharmacy, as provided by Section
465.022(14)(d),
F.S., shall be $100.00.
(4) The
delinquent fee for a pharmacy permit, as provided by Section
465.022(14)(c),
F.S., shall be $100.00.
