Fla. Admin. Code Ann. R. 64B2-18.005 - Certification of Supervising Physicians
(1)
After initial certification and establishment of an initial work arrangement,
any certified chiropractic physician's assistant or chiropractic physician
desiring to supervise, employ or work with a certified chiropractic physician's
assistant other than as initially established, shall make application to the
Board on board approved form DH-MQA 1149, (Rev 06/20), Modifying Supervision
for Certified Chiropractic Physician's Assistant Application, which is hereby
incorporated by reference, and may be obtained from
http://www.flrules.org/Gateway/reference.asp?No=Ref-13348,
or the Board of Chiropractic Medicine, 4052 Bald Cypress Way, Bin #C07,
Tallahassee, Florida 32399-3257, accompanied by the required fees.
(2) As part of the application process, the
Board shall interview the supervising chiropractic physician and the certified
chiropractic physician's assistant about the work arrangement proposed in the
application.
(3) Upon approval of
an application, the Board shall certify the physician or group of physicians to
supervise a certified chiropractic physician's assistant.
(4) Certifications to supervise assistants
shall remain valid for two years and must be renewed
biennially.
