Fla. Admin. Code Ann. R. 64B20-5.0021 - Inactive Status License or Certificate
(1) Any licensee or certified assistant may
elect at the time of renewal to place the license or certificate into inactive
status by filing with the Board a complete application for inactive status and
paying the inactive status fee. For the purposes of this section, a complete
application shall be the Department's renewal notice on which the licensee or
certified assistant affirmatively elects inactive status. The Department will
mail a renewal notice to the address of record of each licensee and certified
assistant at least ninety (90) days before the end of each biennial licensure
cycle, as provided in Sections
456.038 and
456.036, F.S.
(2) Any inactive status licensee or certified
assistant whose license or certificate has been in inactive status for less
than two consecutive biennial licensure cycles may change to active status at
any time provided the licensee or assistant has met the continuing education
requirements of Rule 64B20-6.001, F.A.C., pays the
reactivation fee and, if applicable, the change of status fee as defined by
Rule 64B20-3.0105, F.A.C.
(3) Any inactive status licensee or certified
assistant whose license or certificate has been in inactive status for more
than two consecutive biennial licensure cycles may change to active status at
any time provided the licensee or assistant has met the continuing education
requirements of Rule 64B20-6.001, F.A.C., pays the
reactivation fee, if applicable, the change of status fee as defined by Rule
64B20-3.0105, F.A.C., and files
with the board a complete application. For the purpose of this section, a
complete application shall be form SPA-1 (11/90), the application required for
initial licensure or certification. Form SPA-1 can be obtained from the Board
of Speech-Language Pathology and Audiology, Department of Health, 4052 Bald
Cypress Way, Bin #C-06, Tallahassee, Florida 32399-3256.
(4) Any inactive licensee or certified
assistant who changes from inactive to active status is not eligible to elect
to return to inactive status until the licensee or assistant completes a
licensure cycle on active status.
