Fla. Admin. Code Ann. R. 64B6-4.009 - Inactive and Delinquent Status Fees

The fees for individuals holding a license pursuant to Section 484.045, F.S., shall be as follows:

(1) The fee for an inactive status license shall be $375.00.

(2) The fee for processing a licensee's request to change licensure status at any time other than at the beginning of a licensure cycle shall be $375.00.

(3) The fee for delinquent status shall be $375.00.

(4) The fee for reactivation of an inactive license shall be $150.00.

(5) The fee for retirement status of an active or inactive license shall be $50.00.
